Lack of capacity in our hospitals, fuelled by delayed hospital discharge is a common theme in todays over stretched NHS.
Hampshire Hospitals NHS Foundation Trust (HHFT) worked with CCGs to adopt a new end-of-life service to allow patients to return quickly to the comfort of their own home – significantly reducing the time spent in a hospital bed.
When designing the service, HHFT needed a technology solution which could ensure maximum time with patients with minimal time on back office tasks.
They chose CM2000’s CallConfirmLive! to efficiently schedule visits, manage staff availability, minimise travel between visits, ensure salaried staff are effectively utilised and evidence service delivery.
